Take a quiz to determine if students can identify in which situations, each specific form of identification, may be most useful.

Identification Quiz

  • You are applying for a new job and you are required to fill out a job application. One of the items you need to fill out is your home address. You are nervous and can’t remember it, where could you find your home address so you can copy it onto the job application. (Minnesota ID)
  • You are working at your job, without a job coach. You begin to feel very sick. The supervisor at your job would like to call the school nurse, and let her know that you need to leave work for the day. Where could your supervisor find the phone number to school and call the nurse? (school ID)
  • Tell me a safe way or place to carry your identification. (Wallet, pocket, purse, fanny pack)
  • You are out walking with your friends to Brookdale. You look carefully for cars while you cross the street, but a car turns the corner quickly and hits you. You are knocked unconscious (you cannot see, move or talk). When the paramedics and police come to the accident area, they need to know what your name is, where you live and who they should contact about the emergency. What could the emergency helper look at to find out this information?  (MN ID or emergency information card)
  • What do both your MN ID and your school ID on them, that lets people know exactly what you look like. (photograph)
  • What identification do you carry that has your birthday on it? (MN ID)
  • You have an appointment to see your doctor and s/he would like to know what medications you have been taking. What could your doctor look at that has this information on it? (emergency information card)
  • You have gone to Target with school staff and have gotten separated from the group.  What could a Target employee look at to help you notify your school that you are lost? (school ID)
  • After the Target employee call school, they find out that the group you are with is already back at school, and they are to help you call a taxi to get back to school. Where could they find the address of school so that they can tell the taxi where to go? (school ID)
  • When you go to the bank to cash your social security check, the bank would like you to show them a form of identification that proves who you are. What would you show them. (MN ID)
  • What is on both your school ID and your MN ID that is unique to you? It is almost like a fingerprint in that there are no two alike in the world? (signature)
